How to enter
Entering is easy! Simply fill out our QMHW feedback survey by 5:00pm Wednesday 6 November 2024 and go in the random draw to win.
All survey respondents who complete the feedback survey and opt into the draw by providing their email address or phone number by 5:00pm Wednesday 6 November are eligible to enter. The winner of the $100 Visa gift card will be drawn randomly after the survey closes.
T&Cs apply. This competition is open to Queensland residents aged 18+ only.

Terms and conditions
- The promoter is Queensland Mental Health Week through CheckUP (ABN 56123426111) of Level 2, 36 Russell Street South Brisbane, Qld 4101.
- Entrants into the 2024 Queensland Mental Health Week Feedback Survey Draw will be deemed to have accepted these Terms and Conditions.
- Entry is open to residents of Queensland who are 18 years or older except:
- employees of the Promoter and their immediate family members
- employees of the Queensland Mental Health Commission and their immediate family members
- persons who have breached the terms and conditions of any promotion run by or on behalf of the Promoter (Entrants).
- The entry period commences on Monday 14 October 2024 at 9:00am (AEST) and finishes on Wednesday 6 November 2024 at 5:00pm (AEST).
- In order to enter into the draw, entrants must complete and submit the 2024 Queensland Mental Health Week Feedback Survey between 9:00am Monday 14 October and 5:00pm Wednesday 6 November 2024 and provide their postal address and email address or phone number when submitting their survey entry.
- Entrants may not submit multiple survey entries. Entrants who submit multiple survey entries will be disqualified from the draw.
- There will be one prize winner, who will receive an AUD$100 Visa gift card.
- The winner will be randomly selected from all valid entries no later than 14 days after the survey closes on 6 November.
- The prize winner will be contacted by CheckUP staff members no later than 7 days after the the winner is drawn.
- CheckUP will contact the prize winner via the email address and phone number they provided at the end of the QMHW Feedback Survey to arrange postage of the prize.
- If the prize winner does not respond and accept the prize within 14 days of being notified, they will no longer be eligible to receive their prize and a new prize winner will be randomly drawn.
- Each entrant may only enter one valid entry to this promotion.
- Entries can only be made in an individual’s own name and in their own capacity and no entry can be made for or on behalf of any other person, venture or organisation.
- Entrants’ personal information will not be disclosed to third parties without the Entrant’s consent unless required or authorised by law. View Promoter’s Privacy Policy.
- If prize winners do not provide CheckUP staff with shipping details within 14 business days of being notified, they will no longer be eligible to receive their prize. The Promoter reserves the right in its sole discretion to select alternate winner/s if it determines that the winner/s do not adequately satisfy these Terms.
- Prizes are not transferable. If for any reason a winner does not take the prize, then the prize will be forfeited by the winner and cash will not be awarded in lieu.
- If the Promotion is compromised in any way or is not capable of being conducted as reasonably anticipated due to any reason beyond the reasonable control of the Promoter, the Promoter reserves the right in its sole discretion to modify, suspend, terminate or cancel the Promotion. Notice of any changes to the Promotion, including any consequential changes to these Terms, will be given on the website.
- To the full extent permitted by law, the Promoter is not liable for any injury, damages, expenses or loss whatsoever (including but not limited to indirect or consequential loss) to persons or property arising in any way out of the Promotion.
- Each Entrant indemnifies and keeps indemnified the Promoter against all claims, losses or damages and expenses suffered by the Promoter or any third parties arising out of the breach of these Terms, the conduct of the Entrant in relation to the Promotion or use of a Prize.
- The Promoter’s decision in relation to any aspects of the Promotion is final and binding on all Entrants. No correspondence will be entered into regarding such decisions, the Promotion, these Terms or the results.
- Failure by the Promoter to enforce any of its rights at any stage does not constitute a waiver of these rights.
